2008 Apopka Pop Warner Registration Requirements
Registration will be held at the Bobby Manley Fields.
PAPERWORK NEEDED AT TIME OF REGISTRATION
We will have these 2008 forms available at Registration
1. 2008 Apopka Registration Form
2. 2008 Pop Warner Participant Contract
3. 2008 Apopka Code Of Conduct
4. 2008 Apopka Authorization to Photograph Form
5. Original Birth Certificate
6. $ 85.00 Registration Fee per player or cheerleader.
7. DON'T FORGET TO BRING YOUR FOOTBALL PLAYER.
All Football players must be weighed at registration.
PAPERWORK NEEDED AT TIME OF FIRST PARENT'S MEETING
(1 or 2 weeks prior to first practice)
Pop Warner rules strictly prohibit any player participation without these two documents
1. 2008 Pop Warner Physical Fitness & Medical History Form
2. 3 Copies of Current Report Card (Must be final 2007-2008 report for the entire school year)

1PM - 4PM
PARENT'S, PLAYERS, CHEERLEADERS, COACHES MEETING:
